The following documents are simply a resource developed by the department of Lutheran Schools for the Pacific Southwest District of the Lutheran Church-Missouri Synod. These documents, developed over years of research and using state standards, help teachers, caregivers and parents assess, evaluate, and teach to help children reach age appropriate milestones and benchmarks in their wholistic development.
We provide a copy for your reference.
In addition to the above forms, a copy of the child's birth certificate and up to date immunization records are required by the start of the first day of school.
